Documentation needed to Evaluate A Retirement Plan / Retirement Account

For A Defined benefit Pension Plan (Employer Based) if still employed:
1 Our Defined Benefit Pension form filled out completely Defined Benefit Pension Form.pdf
2 Please provide a copy of the recent Annual Member Benefit Statement (all pages)
3 Please provide a copy of a current pay stub
4 If you work for a non-private employer such as schools, county, state, or other government organization
please provide the pensionable base (annual) pay on the form in the appropriate area
5 If you have purchased time from either Military time or other employed time, please provide the date of
purchase, years purchased, amount paid, and where the money came from (whether separate
or marital money) to purchase the time
6 If you have had any "Breaks in service" while employed, please provide month, day, and year in which
the breaks occurred
7 Is there "Reciprocity" with another employer?
**Dates are very important. When filling out the form be sure to list month, day, and year.**

For a Defined Benefit Pension Plan (Employer based) if in retirement status:
1 Our Defined Benefit Pension form filled out completely Defined Benefit Pension Form.pdf
2 A copy of the most current Direct Deposit statement or a recent Notification of Entitlement and a copy of the retirement papers disclosing election of Survivor Benefits
3 If you had any "Breaks in Service" during employment, please provide month, day and year in which
breaks occurred
4 If you have purchased time from either Military time or other employed time, please provide the date of
purchase, number of years purchased, amount paid, and where the money came from (whether separate or
marital money) to purchase the time

For a Military Retirement Plan (if still enlisted):
1 Our Defined Benefit Pension form filled out completely Defined Benefit Pension Form.pdf
2 A copy of two months LES statements
3 Is enlistment ACTIVE or RESERVE (if RESERVE time, will need Point History statement)

For a Military Retirement Plan (if in retirement status):
1 Our Defined Benefit Pension form filled out completely Defined Benefit Pension Form.pdf
2 A copy of a Direct Deposit statement showing current monthly benefit
3 Were Survivor benefits elected, if so what was Base Annuity amount
4 Receiving disability benefits?

For a Defined Contribution (401(k), Thrift Savings Plan, Deferred Compensation Acct )
1 Our Defined Contribution form filled out completely Defined Contribution Pension Form.pdf
2 ALL Quarterly Statements for the account. Quarterly Statements needed will depend on the following:
a. If you entered the Plan PRIOR to the date of marriage, we will need ALL Quarterly Statements from 1
Quarter PRIOR to the date of marriage THRU current
b. If you entered the Plan AFTER the date of marriage, we will need ALL Quarterly Statements from 1
Quarter PRIOR to the date of separation THRU current
c. If loans were taken out against the account, you will need to provide the amount of the loan, date
date of the loan, and whether the loan is considered marital or separate
